Go to the New Form page:

From there, click “Connect to Notion” to select workspace / pages you want to give Chelly access to, an account will be automatically created if you’re not already signed in on Chelly.
After that, you will be able to create a form and connect it to desired Notion database:

Let’s say we created a form for this “Task List” database:

Chelly will publish a form like this for it:

You may notice that the “Date Created” property from your Notion database is ignored in the form, because it’s a built-in Notion property and should not be touched by a submitter.
Customize the form
In the dashboard you’ll be able to customize the form, like the form title and mark some questions as required:
